Prayer for Spiritual Communion
My Jesus, I believe that you are present in the Most Holy Sacrament.
I love you above all things and I desire to receive you in my soul.
Since I cannot at this moment receive you sacramentally,
Come at least spiritually into my heart.
I embrace you as if you were already there
And unite myself wholly to you.
Never permit me to be separated from you.
Amen.
Prayer to St Martin De Porres (Patron Saint for the Sick)
To you Saint Martin de Porres we prayerfully lift up our hearts filled with serene confidence and devotion.
Mindful of your unbounded and helpful charity to all levels of society and also of your meekness and humility of heart, we offer our petitions to you.
Pour out upon our families the precious gifts of your solicitous and generous intercession; show to the people of every race and every color the paths of unity and of justice; implore from our Father in heaven the coming of his kingdom, so that through mutual benevolence in God men may increase the fruits of grace and merit the rewards of eternal life.
Amen.

Guidelines for Extraordinary Ministry to the Sick
General Qualifications
Special care needs to be taken in the identification, selection, preparation and commissioning of lay pastoral visitors to the sick and homebound. Some are called to this ministry; others are not. The following information is intended to assist parish and ministry leaders in the discernment of that call with those who offer themselves.
QUALIFICATIONS TO SERVE AS AN EXTRAORDINARY MINISTER
Again, it is important that those individuals selected to serve as lay pastoral visitors be chosen carefully. The instruction Immensae Caritatis describes the fundamental qualifications:
The faithful who are special ministers of communion must be persons whose good qualities of Christian life, faith, and morals recommend them. Let them strive to be worthy of this great office, foster their own devotion to the Eucharist, and show an example to the rest of the faithful by their own devotion and reverence toward the most august sacrament of the altar. No one is to be chosen whose appointment the faithful might find disquieting.
In the Diocese of Oakland, the following norms also apply to Extraordinary Ministers of Holy Communion who will visit the sick and homebound:
Living a life in harmony with the teachings of Christ and of the Church If married, must be in a valid marriage according to Church law,
Must have received all the sacraments of Initiation, and
